Improve the reliability of handovers and medication administration in the "wards": solutions for medical and surgical departments

Medical accidents are partly due to defects in communication and information transmission. In order to guarantee patient safety, it is necessary to implement strategies to secure information transmissions. Discover the advice of the SafeTeam Academy.

The healthcare system is undergoing rapid change, and despite technological advances, the workload of caregivers is increasing, thereby increasing the risk of medical errors.

These accidents are partly due to communication and information transmission failures.*

In this context, it is necessary to implement strategies that enable caregivers to ensure the reliability of care and guarantee patient safety.

‍Securinginformation transmission

Transmissions encompass all means of conveying information between members of a care team to ensure continuity of care. This involves enabling the passage of information and acting as an intermediary.

Whether oral or written, communication between the various healthcare professionals is essential and indispensable in patient management, particularly in terms of safety and quality.

Furthermore, communication failures are highlighted in several surveys, particularly the national ENEIS surveys. This is why inter-team handoffs are a real communication challenge. They allow all team members to have a good understanding of the patients and to organize care priorities within the hospitals.

Transmissions can also be leveraged as a focus of work to improve communication and teamwork within hospitals.

To succeed in this work and achieve an effective level of communication, care teams must dedicate time to training on this subject and ideally develop team reflection around innovative training programs.

‍Utilisation del'outil SAED pour des transmissions efficaces

With the primary objective of ensuring patient safety and promoting teamwork, the HAS (French National Authority for Health) highlights a tool that can facilitate communication between healthcare professionals.*

This communication tool is the SAED tool for "Situation Background Assessment Recommendation," which is the French adaptation of the Anglo-Saxon S.B.A.R. tool, "Situation Background Assessment Recommendation." It is a tool that allows healthcare professionals to structure their oral communication with another professional.

This process consists of several standardized steps that help caregivers anticipate the information required in order to enable quality communication between stakeholders regarding a patient's care.

The objective? To prevent adverse events resulting from misunderstandings during handovers, to facilitate the implementation of clear and concise communication, and to avoid omissions.

‍SafeTeamAcademy and transmissions?

Through its innovative training programs, SafeTeam Academy aims to support healthcare professionals and provide them with the tools necessary to improve the reliability of care.

After the check-list, the founders of the SafeTeam Academy made it a priority to work on transmissions and thus developed their second and third modules on this theme.

Their second module stages a transmission in the post-anesthesia care unit and is aimed at healthcare professionals working in these contexts (RN, Nurse Anesthetists, and Anesthesiologists).

The third module is more focused on transmissions on the floors, in the medicine or surgery department, and also addresses the issue of drug administration.

In these modules, healthcare professionals consider several points: the practices to be implemented, the ways to transmit information, strategies for preventing medication errors, and the management of task interruptions.
